Los cambios globales que se han dado a fines del siglo XX, han transformado los hábitos de vida de las sociedades. La mala alimentación basada en lípidos, la continua exposición al estrés, las extensas jornadas laborales, el tabaquismo, la drogadicción, el consumo de fármacos y el sedentarismo, han conducido a la elevación progresiva y desmedida de la tasa de mortalidad por enfermedades del corazón. Argentina no queda al margen de estos cambios sufriendo hoy en día una tasa mayor al 30% de defunciones provocadas por patologías cardiovasculares. Dentro de estos números, el síndrome coronario agudo (daño → isquemia → infarto) juega sin duda un papel central. El infarto es una de las condiciones cardíacas con más probabilidades de desatar arritmias ventriculares malignas y/o muerte súbita cardíaca. Los medios de diagnóstico actuales se basan inicialmente en la búsqueda de alteraciones enzimáticas en el flujo sanguíneo, principalmente Troponina. Sin embargo, los valores predictivos más altos sólo se alcanzan dentro de una ventana de horas posteriores al daño en el miocardio, y aun así no superan el 90%. Asimismo, muchas condiciones de isquemia, etapa previa al infarto, se dan de forma espontánea e imprevista y su diagnóstico tardío conduce inevitablemente al daño permanente. Los métodos más avanzados involucran equipamiento cuyo costo resulta inaccesible para la mayoría de la población. En este contexto, las técnicas electrocardiográficas modernas se presentan como un recurso económicamente alcanzable y a la vez no invasivo. Desafortunadamente, la eficacia de estas técnicas tampoco alcanza un valor suficiente para el diagnóstico temprano y completo, obligando a los especialistas a tomar decisiones sobre un conjunto de estudios, algunos de ellos invasivos. Es por esto, que la correcta evaluación del riesgo cardíaco permanece siendo al día de hoy una búsqueda con desmedidos esfuerzos de investigación científica. El diagnóstico temprano de una patología cardíaca permitiría el desarrollo de un tratamiento adecuado y evitaría a la vez, la medicación innecesaria por falsos positivos. En esta memoria de tesis, una serie de algoritmos novedosos se presentan como una propuesta superadora que busca alcanzar valores de eficacia de diagnóstico suficientemente altos para aportar al no requerimiento de estudios invasivos. En una primera instancia, se recorren diferentes formas para caracterizar la dinámica de la dispersión del vector eléctrico cardíaco, entendida como la proyección de toda la ola de potenciales que se desarrollan en el corazón durante la actividad principalmente ventricular. La dinámica se estudia frente a cambios de la frecuencia del corazón inducidos por una maniobra de inclinación de camilla. Se desarrollan cuantificadores temporales, asociados a intervalos electrocardiográficos, espaciales, basados en transformaciones de componentes principales, y morfológicos, incluyendo áreas, pendientes y amplitudes de la onda de repolarización (onda T). Con estos índices se demuestra que las principales variaciones temporales de la dispersión causadas por el cambio abrupto del ritmo cardíaco (de 1050 ± 150 mseg hasta 810 ± 140 mseg), función recíproca de la frecuencia cardíaca, se observan en la primera mitad de la onda T con reducciones relativas del tiempo de inicio al pico (28 ± 10 mseg) y que el resto de las alteraciones de duración en los intervalos de depolarización y repolarización se deben a la misma. En las energías totales y las áreas, se observan cambios equitativos en ambas mitades, quedando constante la relación que guardan con un 62% de área en la primera mitad, asociada a la repolarización del epicardio. La caracterización se pondera asimismo en jóvenes y adultos para definir las dimensiones normales en cada grupo etario, fuera de las cuales se sugiere un estudio de riesgo cardíaco. Posteriormente, se desarrolla un modelo matemático novedoso que cuantifica la dinámica de la velocidad angular del vector eléctrico cardíaco mediante el procesamiento digital del vectorcardiograma en el cuerpo de los cuaterniones. Dicha velocidad está directamente relacionada a los caminos de conducción eléctrica del miocardio. A lo largo del trabajo, se combina con las velocidades lineales clásicas permitiendo el hallazgo de índices de muy alta sensibilidad y especificidad para el diagnóstico del síndrome coronario agudo. La eficiencia y robustez del método permiten su simple aplicación tanto en infarto, logrando un par sensibilidad/especificidad del 98%/100%, como en isquemia, alcanzando el par 98%/100%. La comparación se realiza contra sujetos sanos. La sensiblidad se mantiene muy alta, sin pérdida de especificidad: 95%/100% cuando se introducen patologías no isquémicas que suelen provocar muchos falsos positivos en los métodos del estado del arte. Además, se propone una mejora al algoritmo de velocidad angular, introduciendo el concepto de red neuronal cuaterniónica con realimentación temporal. Esta red representa un algoritmo que simplifica una parte importante del modelo y permite alcanzar la sensibilidad más alta reduciendo la cantidad de parámetros necesarios para la elaboración de los cardiomarcadores de síndrome coronario agudo. Finalmente, si una arritmia se dispara, esta puede conducir a la muerte súbita cardíaca. Dado que se conocen las características fisiológicas delos mecanismos arritmogénicos, en las últimas décadas se han desarrollado tratamientos basados en agentes farmacológicos que interfieren en la actividad normal de los canales iónicos del tejido muscular del corazón, denominados “drogas antiarrítmicas”. Lamentablemente, si bien muchos experimentos han mostrado la eficacia de estos fármacos en laboratorios, es usual que dicha eficacia no se traslade a la clínica. Esto se debe a que el suministro de estos agentes puede producir efectos secundarios proarrítmicos que complican la condición del paciente. El gold standard actual de medición de riesgo es una metodología basada en el electrocardiograma (ECG) que presenta gran cantidad de falsos positivos. En esta tesis se mostrará un estudio alternativo de la dinámica de la heterogeneidad ventricular bajo los efectos de drogas antiarrítmicas. En un modelo de corazón animal aislado, se mide sobre un tórax artificial una señal símil ECG con treinta electrodos. Con la información provista se construye un modelo espacial y se determinan las alteraciones provocadas por el suministro de la droga. Con este estudio se logra mejorar el análisis de la cardiotoxicidad desarrollando cardiomarcadores que podrían llevarse al vectorcardiograma (VCG) y que complementan eficientemente y de forma descorrelacionada a los índices existentes en el estado del arte. Este resultado podría favorecer el desarrollo de estudios tendientes a alcanzar terapias farmacológicas seguras de control de arritmias que no desaten efectos proarrítmicos, evitando el peligro de muerte súbita cardíaca. Se concluye que la determinación de los rangos de actividad normal, los índices de detección de síndrome coronario agudo y el estudio de la cardiotoxicidad de fármacos, representan un aporte significativo al estado del arte. Dado los valores extremadamente altos de eficacia, se convierten en logros prometedores para la aplicación a registradores de larga duración los cuales pueden ser adquiridos en hospitales de bajos recursos y de esta forma, poder acceder a estudios de rutina que permitan determinar el riesgo cardíaco de forma temprana y eficiente. The global changes that have taken place at the end of the 20th century have transformed the habits of life of societies. Diets that are highly based on fat, sustained exposure to long-term stress, long working hours, smoking, drug misuse, consumption of pharmaceuticals and sedentary lifestyle have led to the progressive and excessive rise in the death rate from heart disease. Argentina is not left out of these changes suffering today a rate higher than 30% of deaths caused by cardiovascular diseases. Within these numbers, the acute coronary syndrome (damage → ischaemia → infarct) plays undoubtedly a central role. Myocardial infarction is one of the cardiac conditions most likely to trigger malignant ventricular arrhythmias and/or sudden cardiac death. The current diagnostic methods are mainly based on the search for enzymatic alterations in the blood flow, mostly Troponin. However, the highest predictive values are only reached within a window of hours after the infarct to the myocardium, and even then they do not exceed 90%. Also, many conditions of ischaemia, stage prior to infarction, occur spontaneously and unexpectedly and its late diagnosis unavoidably leads to permanent damage. The most advanced methods involve equipment whose cost is inaccessible to the majority of the population. In this context, modern electrocardiographic techniques are presented as an economically reachable and non-invasive resource. Unfortunately, the accuracy of these techniques does not reach a sufficient value for early and complete diagnosis, forcing specialists to make decisions about a set of studies, including some invasive ones. This is why the correct evaluation of cardiac risk remains today a search with excessive efforts of scientific research. The early diagnosis of a heart pathology would allow the development of an adequate treatment and avoid, at the same time, unnecessary medication provoked by false positives. In this thesis, a series of novel algorithms are presented as an groundbreaking proposal that seeks to achieve high enough diagnostic efficacy values to contribute to the non-requirement of invasive studies. In a first instance, different forms are traversed to characterize the dynamics of the dispersion of the cardiac electrical vector, which is understood as the projection of the whole wave of potentials that develop in the heart during mainly ventricular activity. The dynamics is studied in response to changes in the heart rate induced by a tilt test maneuver. Temporal indices are developed in terms of electrocardiographic intervals, spatial indices, based on transformations of main components, and morphological indices, including areas, slopes and magnitudes of the repolarization wave (T-wave). With these indices it is shown that the main temporal variations of the ventricular repolarization dispersion caused by the abrupt change of the cardiac rhythm (from 1050 ± 150 msec to 810 ± 140 msec), reciprocal function of the heart rate, are observed in the first half of the T-wave with relative reductions in the onset-to-peak interval (28 ± 10 msec) and that the rest of the duration alterations in the depolarization and repolarization intervals are due to it. In the total energies and the areas, equitable changes are observed in both halves of T-wave, being constant the relation that they keep with a 62% of area in the first half, associated with the repolarization of the epicardium. The characterization is also evaluated in young people and middle-aged to define the normal dimensions in each age group, out of which a study of cardiac risk is suggested. Subsequently, a novel mathematical model is developed in order to quantify the dynamics of the angular velocity of the tip of cardiac electrical vector through the digital processing of the vectorcardiogram in the field of the quaternions. This velocity is directly related to the myocardial conduction pathways. Throughout the work, it is combined with classical linear velocities allowing the finding of very high sensitivity and specificity indexes for the diagnosis of acute coronary syndrome. The efficiency and robustness of the method allow its simple application both in myocardial infarction, achieving a sensitivity/specificity pair of 98%/100%, and in myocardial ischaemia, reaching the pair 98%/100%. The comparison is made with healthy subjects. Sensitivity remains high without loss of specificity: 95%/100% when are including non-ischaemic patho logies that usually cause many false positives in the state-of-the-art methods. Furthermore, an improvement to the angular velocity algorithm is proposed, introducing the concept of quaternionic neural network with temporal feedback. This network represents an algorithm that simplifies an important part of the model and allows reaching the highest sensitivity by reducing the amount of parameters needed for the design of the biomarkers of acute coronary syndrome. Finally, if a pathology triggers an arrhythmia, it can lead to sudden cardiac death. Since the physiological features of the arrhythmogenic mechanisms are well known, in the last decades treatments based on pharmacological agents, called “antiarrhythmic drugs”, have been developed that interfere in the normal activity of the ion channels of the heart muscle. Unfortunately, although many experiments have shown the efficacy of these drugs in laboratories, it is usual that such efficacy is not observed in the clinic. This is because the supply of these agents can induce proarrhythmic side effects that make the condition worse. The current gold standard for cardiotoxicity risk measurement is an electrocardiogram(ECG)-based methodology that presents a large number of false positives. This thesis will show an alternative study of the dynamics of ventricular heterogeneity under the effects of antiarrhythmic drugs. In an isolated animal heart model, an ECG-like signal with thirty electrodes is measured on an artificial thorax. With the information provided, a spatial model is constructed and the alterations caused by the drug supply are determined. With this study, it is possible to improve the assesing of cardiotoxicity by developing cardiomarkers that could be extracted from a vectorcardiogram (VCG) signal and that complement efficiently and in an uncorrelated way the existing indexes in the stateof- the-art. This result could favor the development of studies tending to reach safe pharmacological therapies of arrhythmia control that do not trigger proarrhythmic side effects, avoiding the risk of sudden cardiac death. It is concluded that the determination of normal dispersion ranges, the detection biomarkers of acute coronary syndrome and the cardiotoxicity study represent a significant contribution to the state-of-the-art. Given the extremely high values of accuracy, they become very promising achievements for the application to long-term electrocardiogram (Holter) which can be acquired even in low-income hospitals and thus, provide the entire population with access to routine studies to determine cardiac risk in an early and very efficient way. La interpretación geofísica está en concordancia con los pozos de exploración someros y profundos; además, se integraron la magnetotelúrica y la gravedad a través de la construcción de pseudo-pozos. Finalmente, la integración de datos geofísicos y de pozo permitió la construcción de un modelo estático en Pozuelos, que encuentra en 900 metros de profundidad la zona más profunda del basamento y discrimina ocho litofacies. La exploración y producción de reservorios no convencionales ha surgido como una oportunidad para satisfacer la demanda de hidrocarburos, acelerando así el desarrollo científico y tecnológico en varias áreas de las geociencias, en particular prospección geofísica, que contribuyen tanto al entendimiento del reservorio no convencional como a la caracterización de la geometría y evolución de fractura hidráulica. Los reservorios no convencionales se caracterizan por presentar (respecto de los convencionales) un alto grado de anisotropía, es decir la variación de sus propiedades físicas con la dirección considerada. Por otro lado, dentro de las tecnologías de seguimiento de la evolución de la fractura hidráulica, la tecnología microsísmica es una herramienta útil; sin embargo, debido a su bajo grado de sensibilidad al fluido en poro, no es del todo efectiva en la determinación del volumen de reservorio estimulado, cantidad importante para el evaluar la efectividad de la terminación del pozo y planificar el desarrollo del yacimiento. En estos dos aspectos, los métodos electromagnéticos pueden jugar un importante rol. El objetivo entonces es simular y estudiar la respuesta electromagnética generada al realizar operaciones de fractura hidráulica en un reservorio no convencional. Esto requiere por un lado estudiar su evolución dinámica y por otro lado estudiar el efecto de la anisotropía eléctrica en diferentes escenarios geológicamente plausibles antes, durante y después de la operación de fractura. Con este objetivo en mente, para obtener la solución aproximada de las ecuaciones de Maxwell se han construido simuladores 2D y 3D bajo un método de elementos finitos mixto en tiempo basado en elementos conformes de Nédélec para la discretización espacial y paso de tiempo leapfrog para la discretización en tiempo. Además, se provee un análisis de estabilidad numérica de dicho método numérico. Los resultados indican que la señal electromagnética, de largo período, está en el rango medible y las conclusiones se centran en el valor agregado de los métodos electromagnéticos a la determinación del volumen de reservorio estimulado y a la respuesta de cada campo eléctrico y magnético a cada escenario anisotrópico propuesto. En base a esta respuesta se define la sensibilidad de los campos a la configuración transmisor-receptor, la sensibilidad de los campos a la ubicación de los receptores y la sensibilidad de los campos al tipo de anisotropía o discriminación entre dos tipos de anisotropía. Finalmente, si bien se ha estudiado esta aplicación a reservorios hidrocarburíferos no convencionales, el análisis tiene aplicación a problemas de recuperación secundaria y a otro tipo de yacimientos caracterizados por la dinámica hidrogeológica de rocas anisotrópicas saturadas con salmueras como pueden ser los yacimientos de litio. The exploration and production of unconventional reservoirs has emerged as an opportunity to satisfy the demand for hydrocarbons, thus accelerating scientific and technological developments in various areas of geosciences, in particular geophysical prospecting, which contributes to the understanding of the unconventional reservoir and to the characterization of the geometry and evolution of hydraulic fracturing. The unconventional reservoirs are characterized by presenting (with respect to conventional ones) a high degree of anisotropy, that is, the variation of their physical properties with the direction considered. On the other hand, within the technologies for monitoring the evolution of hydraulic fracturing, microseismic technology is a useful tool. However, due to its low degree of sensitivity to pore fluid, it is not very effective to determine the stimulated reservoir volume, which is an important quantity to evaluate the effectiveness of the completion of the well and plan the development of the reservoir. In these two aspects, the electromagnetic techniques might play an important role. The objective is to simulate and to study the electromagnetic response generated when performing hydraulic fracturing operations in an unconventional reservoir. This requires, on one hand, to study its dynamic evolution and, on the other hand, to study the effect of electrical anisotropy in different geologically plausible scenarios before, during and after the fracture operation. With this objective in mind, to obtain the approximate solution of Maxwell's equations, 2D and 3D simulators have been built under a time-mixed finite element method based on Nédélec conformal elements for spatial discretization and leapfrog time step for discretization in time. In addition, a numerical stability analysis of this numerical method is provided. The results indicate that the long-term electromagnetic signal is in the measurable range and the conclusions focus on the added value of electromagnetic methods to the determination of the stimulated reservoir volume and to the response of the different electric and magnetic fields to each anisotropic scenario. Based on this response, the sensitivity of the fields to the transmitter-receiver configuration, the sensitivity of the fields to the location of the receivers, and the sensitivity of the fields to the type of anisotropy or discrimination between two types of anisotropy are defined. Finally, although this application has been studied for unconventional hydrocarbon reservoirs, the analysis has application to waterflooding problems and to other types of reservoirs characterized by the hydrogeological dynamics of anisotropic rocks saturated with brines, such as lithium reservoirs.
